Pretty sure engineers didn't need to find out if it would work. They knew it would work when they designed it. That's why they are engineers is there job to know that something's going to work before it's built.
You showed a planetary gear set , indeed where two gears are in mesh inside a sun gear the direction is the same but when you introduce a third the direction changes so destroys your same direction line, it’s to reduce size , the planetary set is a perfect example , huge torque multiplication for very small size to conventional gearboxes, plus axial loads are pushed inwards not outwards so the shaft being the strongest point less need for bearings to handle the loads so less overall mass also

No engineer has ever asked if the gear can be put on the inside. The answer is obvious to anyone who isn't an idiot
The main reason for a internal gear setup is mostly because of necessity. Examples would be location and size restrictions, a planetary gear setup or a specific way the gear setup needs to work
