One thing i'd like to draw more attention to, is that a very in-depth injury system functionally necessitates a very in-depth care system. A huge part of your time spent in this game is spent *helping* the expie. Bandaging wounds, tending to bruises etc. Your job is to keep it as healthy, happy and fed as you can. Of course, a lot of the time you won't succeed because of the horrible situation the game throws you into, but the emphasis is always on it being worth to try your best anyway. It's perhaps the most direct with this message in the "Let's not give up just yet" screen that pops up when the game gives you a second chance. Personally, i never had any issue with projecting myself into the expie, and i've found that seen this way the game turns into somewhat of an ultimate self care sim.

i think the last stand mechanic really goes against the idea of the game being sadistic the word choise adds to it too the game saying to you the player "lets not give up just yet" really shows the mentality of the game and the theme its trying to go for

I like this game, but man i do find it hard to play at times with how awful i feel for hurting the adorable little expies. They do deserve better, and i really hope the Dev does adds a good ending.

Perhaps in a strange way it's like Frostpunk: yes you could drive the game in a terrible direction but surely you want a perfect ending? Where you did your best? No unneeded suffering? In this case as a handler, surely you should be rewarded for building trust and treating them well - not just because it's efficient. Though you could be as soulless as you wish if keeping the creature from being depressed is simply "how will we achieve our goal?"
