I think a better entry for Howard Stern was his interview with Dana Plato (the day before her suicide, btw).
In 1971 in Germany, there was a talkshow with Nikel Pallat, a member of the band "Ton Steine Scherben". During the show, he got really annoyed at some point, so he whipped out an axe and started hitting the table with it

Letterman has many strengths. But his interviews with women were horrendous and horrifying on many levels.
There are many times when celebrities are set up by triggering interviewers. You can't always condemn them when they're on the spot.
Lindsay has been through so much in her life, it's so inappropriate of David Letterman to bring it up. Thankfully, she now leads a (hopefully) happy life with her husband and child.
