There's another way to get this ending, by failing a certain timed encounter. Only a complete fool would fail to notice the turn counter and waste all their time, which is why I know about it.

If you quick save during this cinematic and reload you and your party are all transformed mindflayers in some weird area, but you can fast travel back to camp and keep playing the game. You and the party have all the skills and abilities of the full mindflayers you become at the end of the game (if you choose those options). I didn't finish the playthrough this way but I did keep going a bit and the game seems to work still.

If this game doesn’t win game of the year I’ll be surprised. Over 200 hours in and multiple playthroughs and I’m still seeing new things happen in this game
