Ohm survives because he accepts the brutal truth of what happened, which breaks the entity's psychological hold over him. While the chains were physically pulling him, his realization that Mal—not a demon—killed Fiona provided the mental clarity needed to physically struggle and unhook himself from the mechanism. The director explained that the entity thrives on deception and "hokum"; once Ohm stopped lying to himself about the reality of the murder, the supernatural connection severed. He was left with the physical scars of the chains as proof that the encounter was not merely a hallucination.
