I went to Recording Engineering school years ago and they specifically went over watching out for those XLR adapters. They basically said that if we're ever in a studio and find one of those, to stop everything, tell the talent not to touch anything, don't plug in any instrument or mic, and speak to whoever runs the studio. They strongly suggested that any venue or studio that has one of those adapters should be basically blacklisted going forward. The risk of making any metal-shelled piece of kit electrically hot could be deadly, not to mention it's a great way to cook your very expensive gear!
