In 2006, California, legendary climber Todd Skinner began a route he had successfully completed before. He had ordered a new climbing harness for the trip—but it never arrived. His partner noticed the old harness was badly worn and warned him not to use it. Skinner chose to continue anyway. As he climbed, the harness scraped repeatedly against the rock, thinning with every movement. When he finally shifted his full weight onto it, a single loop failed. Todd Skinner fell nearly 67 feet. He died instantly. This tragic accident became a stark reminder that in extreme environments, small equipment failures leave no second chances.
