Can confirm when I do this in GTA my intent isn't to just lightly bruise the NPC pedestrians

To be fair "I'm too dumb to realise the consequences of my actions" is a valid defence when it comes to intent.
A permanent ban from ever being able to drive should go along with any other charges. Driving is a privilege, not a right.

society really is built around cars to the point that killing someone with a car will get you a much lighter sentence.
